My client reports a high deductible health care insurance, cannot afford her deductible, and is otherwise SMHW eligible. How do I bill SMHW for services?
The
clients insurance must be billed first. Do not bill SMHW until the Explanation
of Benefits (E.O.B.) is available. In
the comments section of the MOHSAIC form, include what insurance paid per CPT
code and descriptive line item. For example, “Insurance paid zero on office
visit, CPT 99203. Insurance paid $86.00 of bilateral breast ultrasound CPT
76641. Maintain a copy of the E.O.B. in the client’s record for Quality
Monitoring.
