What do you do if the patient’s bed is “down” for a valid reason during the observation (e.g., turning patient, patient is having an abdominal x-ray or a procedure that requires a supine position)?
Ideally, you would return when the procedure was completed to make the observation. If this is not possible, you would record N/A because a valid observation was unable to be made.
