What do I need to know about filling out the inspection form?

The inspection report should be completed fully, including all address blanks, signature blanks and inspection “time in-time out” blanks. Food temperatures are to be recorded and reported on the form as well as the menu. The inspection time shall be a minimum of 30 minutes for food preparation sites and an unobligated timeframe for service sites. The inspection report is to be completed on the DHSS food establishment inspection report E6.39.
