What do I send in with my new Type 3 permit applications?

If you are a Type 2 and you are submitting the applications of your newly trained Type 3’s, you must send us three (3) items for each applicant:  1) A completed and signed Type 3 permit application, 2) a copy of their final written exams, and 3) a copy of the ten self-administered tests and operational checklists for the course that they have just completed.

If you are a Type 3 applicant and your training was performed by a training academy, you will need to contact the training academy to obtain the necessary paperwork for submittal.  
Be sure to retain copies of everything sent to us until you have received your permit from us. This will help relieve any aggravation associated with items getting lost in the mail.
