Can I fax or email paperwork in to your office?

Yes, you may fax your maintenance reports, requests or applications. If faxing any paperwork, leave a number so that we may contact you if there is an error in transmittance. Faxing takes the place of mailing and is considered an original, so do not send hard copies via mail if you have faxed them. Ensure the legibility of each page before faxing. Fax number is 573-840-9139.

If you have a scanner, you may scan in your maintenance reports, requests or applications and email them to our office at breathalcohol@health.mo.gov.  As with faxing, email takes the place of mailing and is considered an original, so do not send hard copies via mail if you have emailed them.  Ensure the legibility of each page before emailing.  Remember that all forms still require signatures, so always make sure that all forms are signed and completely filled out before scanning them and emailing.
