During the proposal process, the Department will mail you Medicaid enrollment forms to complete and return to the Department. If the proposal is approved by the Department and a contract is executed, these enrollment forms are forwarded to MHD for processing. MHD notifies the Provider and the Department by letter of the Medicaid number(s) when the enrollment has been finalized.
